The form is a fillable PDF, however, filling in the blanks will not submit the form to your local branch of the NAACP. After completing all the blanks, you must print the form and sign it. You can then submit it by scanning it and emailing it to, legalredressjcnaacp@gmail.com, or mailing it to the address listed on the form itself.
